DODD APPLICATION

Independent Providers MUST have a DODD provider number prior to being certified.
Prior to class the following must be submitted to Jes Schafer at JSchafer@nursingnavigators.org
- Evidence of High School Diploma or GED
- A copy of a state Issued ID
- Page 1 of the DODD application (page 2 of the application will be completed in class)

For Initial Category 2 & 3

To receive a Category 2 or 3 Certification you must have a valid Certification 1 in good standing and it MUST be obtained and entered into the DODD system prior to receiving additional certifications.

For Renewal Classes

Your certification must be renewed every year and requires the following:

The renewal must be completed by the certification expiration date to continue administering medications and treatments. If expired, you have up to 60 days after expiration to complete renewal requirements, however, you may not continue to administer medications or treatments until recertified.
